I, Yekaboty Balajee Rao (old name), R/o H.No: 7-204-1, Gowlipet Street, Adoni - 518301, Kurnool District, Andhra Pradesh, hereby declare that I have changed my surname from Yekaboty to Yekabote. Henceforth, I shall be known as Yekabote Balajee Rao for all future purposes.
