I, Shilpa Ruhela D/o Rajendra Kumar Ruhela, R/o 588, Chhipiwada, Pilkhuwa, Dhaulana, Hapur, hereby declare that my father’s name is wrongly mentioned in my passport as Rajender Kumar Ruhela. The correct spelling is Rajendra Kumar Ruhela as per all documents.
