I, Rupa Bhattacharyya, D/o Subhas Chandra Majumder, W/o Prasanta Bhattacharyya, residing at 18/D, Vidyasagar Lane, P.O. Abagram, P.S. Uttarpara, District Hooghly, West Bengal-712246, do hereby declare that I shall henceforth be known as Rupa Majumder Bhattacharyya. This change has been declared before the Executive Magistrate, Serampore Court, Hooghly, West Bengal, vide Affidavit Number 10128 dated 16-06-2026. Rupa Bhattacharyya and Rupa Majumder Bhattacharyya are one and the same person.
