I, SHILPI Sarma, D/o Arati Sharma, W/o Sanjib Sarma, residing at 78, Puipukur Road, Khudirampally, P.O. Kazipara, P.S. Barasat, North 24 Parganas, PIN-700124, West Bengal, do hereby declare that I have changed my name and shall henceforth be known as Shilpi Sharma. This declaration has been made before the 1st Class Judicial Magistrate, Barasat Court, District North 24 Parganas, vide Affidavit No. 17AC144534 dated 16-03-2026. Shilpi Sarma and Shilpi Sharma are one and the same person.
