I, Debarya Chowdhury, son of Debjyoti Chowdhury, residing at 30E, Hare Krishna Sett Lane, Kolkata-700050, do hereby declare that my name has been incorrectly recorded in certain documents. As per affidavit dated 18-06-2026 before the First Class Judicial Magistrate, Lower Division Court, I shall henceforth be known as Debarjyo Chowdhury instead of Debarya Chowdhury. Both names refer to the same and identical person, and the corrected name shall be used in all future records.
