I, SK Shamir Uddin, son of SK Sajahan, do hereby declare that my father’s name has been wrongly recorded in different documents as “SK Shahjahan” in academic certificates and as “Shaikh Shajahan” in passport records. An affidavit has been executed on 11-06-2026 before the Learned Judicial Magistrate at Alipore Court (No. 50283) confirming that SK Sajahan, SK Shahjahan, and Shaikh Shajahan are one and the same person. All concerned are requested to treat the correct name as SK Sajahan for all official purposes.
