Chhapikul Islam, son of Afjal Mia, resident of Village & P.O. Balakandi, P.S. Dinhata, District Cooch Behar, has declared through an affidavit that his name was mistakenly recorded as "Chhapikul Hak" in Aadhaar Card No. ****0995. He has officially established his correct name as "Chhapikul Islam" through Affidavit No. 12616 sworn before the Executive Magistrate Court, Dinhata, on 30 June 2026.
