This is to inform that I, Sanjay Mishra, S/o Gouri Shankar Mishra, residing at Mahananda Para, Chanda Park, Kali Mandir Road, House No. 8, Ward No. 4, Pin 734005, District Darjeeling, West Bengal, do hereby declare that I have changed my name from Sanjay Mishra to Sanjay Kumar Mishra. Henceforth, I shall be known as Sanjay Kumar Mishra for all purposes. This declaration has been made vide Affidavit No. 03 sworn before the Notary Public at Siliguri dated 17-06-2005, and both names are identical and refer to the same person. West Bengal
