Jai Prakash, S/o Ram Ji, R/o House No. 266, Street No. 6, Savita Vihar, Mukandpur, New Delhi-110042, has officially changed his name from Jai Prakash to Jai Gaur. He has declared that he shall hereafter be known as Jai Gaur for all future purposes and in all official and personal documents.
