I, Deepanshi, D/o Kuldeep Kumar, born on 25/07/1997 and residing at A-24, Shish Ram Park, Uttam Nagar East, New Delhi-110059, hereby declare that I have changed my name to Deepanshi Tongaria vide affidavit dated 30th June, 2026 executed at Gurgaon. Henceforth, I shall be known as Deepanshi Tongaria for all purposes.
