I, Sheik Rahimunnisa Begum W/o Sheik Khaja Syruddin, R/o Kushaiguda, Medchal–Malkajgiri District, Telangana, hereby declare that I have changed my name from “Sheiak Rahimunnisa Begum” to “Sheik Rahimunnisa Begum” for all future purposes. Both names belong to one and the same person.
