This is to inform that the deponent Abdur Rouf, S/o Zainul Abedin, resident of Vill & P.O. Mirual, P.S. Raiganj, Dist. Uttar Dinajpur, West Bengal, declares that due to a clerical mistake his daughter’s name was wrongly recorded as “Raicha” in the Birth Certificate (Regd. No. 351 dated 18.04.2011) issued by the Registrar (Birth & Death), Govt. of Madhya Pradesh. The correct name of the child is “Raisha Hoque” (D.O.B. 31.03.2011). It is declared that Raicha and Raisha Hoque are one and the same person as per affidavit No. 39AA 439068 dated 10.09.2025 before Judicial Magistrate First Class, Raiganj.
